Role Summary
- Support the referral of patients from primary care onto our programmes.
- Work closely with Engagement Leads and internal teams.
- Monitor referral inboxes and enter referrals into the core system swiftly and accurately.
- Check referral data for accuracy and eligibility, flagging issues as needed.
- Liaise with GP practices as required.
- Strong focus on achieving performance targets.
Responsibilities
- Enter referrals from primary care into secure contract inboxes and core system quickly and accurately.
- Review referrals for eligibility and missing information; flag for follow-up.
- Meet or exceed internal targets for entering, reviewing, and responding to referrals within 2 working days.
- Track inbox and related activity.
- Build and maintain strong relationships with Engagement Leads and internal staff.
- Follow up with Engagement Leads or primary care staff for missing information.
- Support other data entry tasks as needed.
- Maintain accurate records.
- Support participant outreach to provide information about the programme and service, as discussed with Line Manager.
- Other duties as agreed with Line Manager.
